Regional Training and Regional Camps Registration Forms
*Please be sure to indicate which Camp you are attending (Winter, Spring, or Fall) from the dropdown menu at the top of the form.*
Regional Trainings taking place on The Ohio State University campus require a waiver form to be completed and submitted to the front desk of the building on the day of training.

Kyu and Dan Exam and Pre-Exam Forms
Please fill out the top half of the exam form completely. Doing so in a PDF reader program will populate the Examiner’s portion of the form automatically.
All 1 kyu and above must purchase an ISKF Passport.
DAN REGISTRATION INFORMATION
The original request form must be accompanied by one passport-sized photograph stapled to each Dan registration form along with the Dan Testing form and the appropriate testing and registration fees. These materials should be sent to the Mid-America Regional Office.
Testing and Registration fees should be made payable to the MAK (Mid-America Karate, Inc.).
All Dan ranking is registered with the International Shotokan Karate Federation. The ISKF will evaluate all registration requests for approval and issuance of registration numbers. Dan certificates will be forwarded from the ISKF Headquarters to the Regional Director, who will then deliver them to the student or their instructor.
PLEASE BE SURE TO SEND THE APPROPRIATE FEES ALONG WITH YOUR DAN EXAM/REGISTRATION FORMS.

Payments
Payment for fees and donations can be made via check or money order made out to Mid-America Karate, Inc. and mailed with the corresponding form to the address above, or electronically via Paypal. When using Paypal please select the purpose of your payment from the dropdown menu.
Note: If you need to mail a form and do not have access to a printer, many public library branches offer low cost or no cost printing.
